Chiropractic care is self-referral. Still, a trusted professional making the connection can sometimes be the difference between a patient seeking care and continuing to wait.

Each form is fillable on a computer and designed to print clearly. The referral interests are tailored to each profession so you can share what is most relevant without turning the form into a clinical intake.
Protecting patient information
Fill. Print or fax securely.

Complete the relevant form with the patient and obtain their written consent.

Securely fax the completed form to (587) 951-1058, or print it and give it directly to the patient.

Do not send completed referral forms or patient health information to RDSS by ordinary email.
A referral does not replace RDSS assessment or guarantee that a particular service will be appropriate. GLA:D Hip and Knee programming is included as an area of referral interest.
